What Is Medical Botox?

The FDA-approved treatment protocol consists of 155 units injected across 31 standardized sites in the forehead, temples, scalp, neck, and shoulders every 12 weeks.

Most major commercial insurance plans approve the 155-unit protocol when medical criteria are met.

Medicare Standard for Chronic Migraine

Medicare typically follows national coverage guidelines that define chronic migraine as:

  • 15 or more headache days per month
  • At least 8 of those days meeting migraine criteria
  • Failure or intolerance of prior preventive medications

When these criteria are documented, Medicare generally covers Botox using the FDA-approved 155-unit dosing protocol every 12 weeks.

Coverage requirements may vary slightly by Medicare Administrative Contractor (MAC), and prior authorization may be required. Our team assists with documentation and insurance verification to help streamline the approval process.

Botox relaxes the masseter muscles responsible for clenching, reducing tension and associated pain.

Results typically last 3–4 months. Insurance coverage for TMJ varies and depends on documentation of medical necessity.

Hyperhidrosis (Excessive Sweating)

Botox is FDA-approved for severe underarm sweating and may also be used for:

  • Palms
  • Soles of the feet
  • Facial sweating

Botox blocks the nerve signals that stimulate sweat glands, significantly reducing perspiration in treated areas.

Results typically last 4–6 months. Insurance coverage may be available when medical criteria are met.
